- Arrangement Form | During each cremation arrangement conference our cremation specialists will be asking for the deceased’s vital information. Once collected, this information will be used for the completion of the Death Certificate, notification to Social Security and claim forms for Veterans. This form will help you get that information organized.
- Oklahoma Cremation Permit Application (CME-19P) | Use this convenient online link to access & complete your cremation Permit application for the State of Oklahoma. This on-line form may be fully completed before printing for signature & faxing to our Cremation Society office.
- Cremation Authorization Form | The Cremation Society provides our families a Worry Free Guaranteed cremation process. A big part of that cremation guarantee process is allowing the families we serve to learn more and become better acquainted with cremation. This Cremation Authorization may be downloaded and printed for stress free consideration.
- Credit Card Authorization Form | For the convenience of our families, we accept all major credit cards. Whether you are making your arrangements online through our website or you are completing the required forms and authorization from out of town, this simple form will allow you to make payment to us via fax or mail.
- Obituary Worksheet | Many families take pride in writing a meaningful life story for their loved one. Our easy to follow Obituary Worksheet will allow you to share that story with your friends and family. Our Cremation Specialists can help you compose and submit that tribute to your favorite newspaper, post as a link on your Facebook page and share from our beautiful Online Memorial Tribute section on our website.

- Register Your Cremation Wishes FORM.pdf | Download an inter-active .pdf Cremation Registration Form here. Use your computer to type and complete your cremation wishes for printing immediately. Return the registration form by fax, or U.S. mail.
- Oklahoma National Cemetery Pre-Registration Form | All Veterans with honorable discharges and at least one day of active duty, and their spouses, are entitled to free cemetery property, headstones, grave opening and closing, and perpetual care at any National Cemetery.
